2017 PLAYER MEDICAL RELEASE FORM/ PARENT VOLUNTEER FORM

Just a reminder that all parent volunteers must complete a Little League volunteer form, and authorize the league to submit to a background check before participating in any BELL practice, game or event. If you have not yet completed and submitted your volunteer application, you can DOWNLOAD IT HERE and give it to your players manager or coach, or email the completed form to .

(Note: first-time volunteers must provide their social security number for the mandatory background check)

 

Also, all registered players must have a completed Little League Medical Release form on file with BELL prior to participating in any practices or games. If you have not already completed your player(s) Medical Release form and submitted it to your team manager, you can DOWNLOAD IT HERE and submit it to your players manager or coach.
